The department shall license each applicant who the board certifies:
- Has completed the application form and remitted the required fees
- Submits three letters of good moral character
- Is 18 years of age
- Is a graduate of an accredited high school or its equivalent
- Submits one photograph
- Meets either option below:
- Has met the requirements of the training program; or
- Has a valid, current license as a hearing aid specialist or its equivalent from another state and has been actively practicing in such capacity for at least 12 months; or Is currently certified by the National Board for Certification in Hearing Instrument Sciences and has been actively practicing for at least 12 months.
- Has passed the licensure examination, which is the International Licensing Examination (ILE) for the Hearing Instrument Dispenser.
- Has submitted proof of taking a two hour course relating to Florida Laws and Rules
- Has submitted proof of taking a two hour course on the prevention of medical errors
If licensed in another state, territory, or jurisdiction of the United States, must have license verification form sent directly to the board office from that office that issued the license or certification.
If an applicant fails to complete the application within 12 months of its receipt in the Board office, he shall be required to apply as an initial applicant.
Applicants applying as actively practicing in another state, without National Board Certification:
- Applicant submits the Application for Licensure by Examination along with the appropriate licensure fees listed on the application instructions. Applications can be found on the Board website at: www.floridashearingaidspecialists.gov.
- All credentials listed on the application instructions must also be submitted to the Board Office to complete the application.
- Documentation to prove ‘actively practicing’ must be submitted with credentials. Applicants must submit 2 contracts per month for 12 of the last 18 months immediately preceding the application.
- Board reviews application and credentials and schedules the applicant for the next available licensure examination.
- Applicants are mailed notification that they have met Board Office requirements for the examination. Along with this notification, the applicant is also mailed the IHS Application Form.
- This application must be completed and mailed to the IHS along with their $195.00 examination fee by the deadline listed on the form. Failure to complete and submit this form and fee by the deadline will result in the applicant being unable to sit for the examination.
- Applicant sits for licensure examination. Applicants will be notified within 4-6 weeks of their pass/fail status.
- Passing applicants will be issued a Hearing Aid Specialists License.
- Failing applicants will be notified of their unsuccessful examination attempt and can apply for future examinations with the Board Office.
Applicants applying as actively practicing in another state, with National Board Certification:
- Applicant submits the Application for Licensure by Examination along with the appropriate licensure fees listed on the application instructions. Applications can be found on the Board website at: www.doh.state.fl.us/mqa/hearingaid.
- All credentials listed on the application instructions must also be submitted to the Board Office to complete the application, including the proof that they are Nationally Board Certified (NBC-HIS).
- Documentation to prove ‘actively practicing’ must be submitted with credentials. Applicants must submit 2 contracts per month for 12 of the last 18 months immediately preceding the application.
- Board reviews application and credentials and issues the Hearing Aid Specialist License; applicants with NBC-HIS certification are exempt from the state licensure examination.
